Private Pay Fee Schedule
We are pleased to offer one flat rate for all services for seniors and individuals with physical disabilities. This includes all taxes and insurance. There is a minimum of 4 hours per day or per week. Our rate is modified for weekend service.
Faithful At Home Care is approved for the following waivers:
- Act 150
- Attendant Care
- COMMCARE
- Independence
- OBRA
- PDA (Aging Waiver)
Faithful At Home Care is also approved under the Office of Developmental Programs for the following waivers:
- Consolidated Waiver
- MR Base Services
- P/FDS Waiver
- Community Living Waiver
For Office of Developmental Programs waivers, we can provide home and community habilitation, companion services, respite, and community participation supports.
